What Is a Biophysical Profile Ultrasound and Why Is It Done

Late pregnancy comes with a lot of monitoring, especially when a clinician wants a closer look at how a baby is doing before birth. One test that may come up is a biophysical profile ultrasound, often called a BPP.


A biophysical profile is a prenatal test used to check a baby’s well-being in the uterus. It combines ultrasound observations with, in many cases, a nonstress test that records the baby’s heart rate. The goal is to look for reassuring signs that the baby is getting enough oxygen and tolerating the pregnancy well.



What a biophysical profile ultrasound checks


A biophysical profile looks at several signs of fetal health. These signs are based on the idea that a baby who is getting enough oxygen is more likely to move, practice breathing, have good muscle tone, have enough amniotic fluid, and show healthy heart rate patterns.


A full BPP usually checks five areas:


  1. Fetal breathing movements

  2. Fetal body movements

  3. Fetal muscle tone

  4. Amniotic fluid volume

  5. Fetal heart rate response, usually through a nonstress test


Each area gives a small but useful piece of the picture. Together, they help the care team decide whether the baby appears well or whether more monitoring, testing, or delivery may need to be considered.


Some offices perform a modified biophysical profile instead of a full one. This usually includes a nonstress test and an ultrasound measurement of amniotic fluid. A modified BPP can be useful when the clinician wants a quicker check but still needs meaningful information.


Why a biophysical profile is done


A BPP is most often done in the third trimester, when the baby is developed enough for movement, breathing practice, and heart rate patterns to be assessed. It is usually ordered when the care team wants extra reassurance that the baby is doing well.


Common reasons include:


  • Decreased fetal movement


If the baby seems to be moving less than usual, a BPP may help check for reassuring signs.


  • Pregnancy that goes past the due date


As pregnancy continues beyond the due date, the placenta may need closer monitoring.


  • High blood pressure or preeclampsia


These conditions can affect blood flow to the placenta and may require closer fetal surveillance.


  • Diabetes during pregnancy


Gestational diabetes or preexisting diabetes can increase the need for monitoring, depending on how the pregnancy is progressing.


  • Concerns about fetal growth


If the baby appears smaller than expected or growth has slowed, a BPP may be part of ongoing assessment.


  • Low amniotic fluid


Amniotic fluid is one marker of placental function and fetal well-being.


  • Multiple pregnancy


Twins or higher-order pregnancies often require closer follow-up.


  • Certain maternal health conditions


Kidney disease, autoimmune conditions, some heart conditions, and other medical issues can lead to more frequent fetal monitoring.


  • Prior pregnancy complications


A history of stillbirth, placental problems, or severe growth restriction may lead to extra testing in a later pregnancy.


A BPP may also be ordered after an abnormal or non-reassuring result on another test. In that case, the BPP helps the care team gather more information before deciding what to do next.


What happens during the test


A biophysical profile is noninvasive. It does not involve needles, injections, or radiation. The ultrasound portion uses sound waves to create images of the baby, placenta, uterus, and amniotic fluid.


The test usually takes place in an obstetric office, ultrasound center, hospital outpatient unit, or labor and delivery triage area.


The ultrasound portion


During the ultrasound, the patient lies on an exam table while a sonographer or clinician places gel on the abdomen. A handheld device called a transducer moves across the belly to capture images.


The ultrasound checks for:


  • Baby’s breathing-like movements

  • Larger body movements, such as kicks or rolls

  • Flexing and extending of arms, legs, hands, or the spine

  • Pockets of amniotic fluid around the baby


Fetal breathing movements do not mean the baby is breathing air. Before birth, oxygen comes through the placenta and umbilical cord. These movements are practice motions involving the diaphragm and chest.


The nonstress test portion


A full BPP often includes a nonstress test, also called an NST. During an NST, monitors are placed on the abdomen to track the baby’s heart rate and contractions, if any are present.


The care team looks for heart rate accelerations. These are brief rises in fetal heart rate that often happen with movement. In many pregnancies, accelerations are a reassuring sign.


The word “nonstress” means the test does not place stress on the baby. It simply observes the heart rate pattern over time.


How the BPP score works


A full biophysical profile is usually scored out of 10 points. Each of the five areas receives either 2 points or 0 points.

A score of 8 or 10 is usually considered reassuring, depending on the amniotic fluid result and the full clinical picture. A score of 6 may be considered borderline and often leads to repeat testing or closer monitoring. A score of 4 or lower is more concerning and may lead to further evaluation or discussion about delivery, especially if the pregnancy is near term.


Scoring can vary slightly by practice, and results are never interpreted in isolation. Gestational age, maternal health, fetal growth, medications, and the reason for testing all matter.


A BPP is not just a number. It is one part of a larger decision that includes the pregnancy history, current symptoms, and the baby’s overall pattern of well-being.

What each part of the test can suggest


Each BPP category reflects a different function. When a baby has normal oxygen levels and a healthy nervous system response, these activities are more likely to appear during the test window.


Fetal breathing movements


Fetal breathing movements are rhythmic motions of the chest or diaphragm. These practice movements can come and go. A baby may not show them constantly, which is why the test allows time for observation.


If breathing movements are seen, that is reassuring. If they are not seen during the test, it does not always mean something is wrong. The baby may be sleeping, or the observation window may not capture that activity.


Fetal body movement


The ultrasound looks for larger movements, such as rolling, stretching, or moving limbs. Babies have sleep and wake cycles, so movement can vary.


Reduced movement during the test may lead the care team to continue observation, repeat testing, or combine the result with NST findings.


Fetal muscle tone


Muscle tone refers to flexion and extension. For example, the baby may open and close a hand or bend and straighten an arm or leg. Good tone is generally reassuring because it reflects healthy neurologic activity.


Amniotic fluid level


Amniotic fluid gives an indirect clue about longer-term placental function. Low fluid can happen for several reasons, including ruptured membranes, placental issues, certain medications, or fetal kidney and urinary tract concerns.


Fluid level often carries special weight. Even if other parts of the BPP look good, low amniotic fluid may prompt closer follow-up.


Fetal heart rate response


The NST portion looks at how the baby’s heart rate behaves over time. A reactive NST, meaning the heart rate shows expected accelerations, is generally reassuring. A nonreactive NST does not automatically mean the baby is in danger. Sleep cycles, medications, and gestational age can affect the result.


Benefits and limits of a BPP


A biophysical profile has several strengths. It is noninvasive, widely used, and provides real-time information about fetal activity, fluid level, and heart rate patterns. It can help guide decisions in pregnancies that need closer monitoring.


Still, it has limits.


A BPP shows what is happening during a specific window of time. A baby may be asleep during part of the test. Some results fall into a gray area. The test may lead to more monitoring, and in some cases it may lead to delivery recommendations when the care team believes the baby may do better outside the uterus.


It also cannot predict every emergency. Placental problems, cord accidents, and sudden changes can sometimes occur even after reassuring testing. That is why symptoms and fetal movement still matter.


How a BPP differs from a regular ultrasound


A regular prenatal ultrasound may focus on anatomy, growth, placenta location, or due date confirmation. A BPP focuses on fetal well-being near the time of the test.


The same ultrasound equipment may be used, but the purpose is different. A growth scan asks, “How big is the baby, and how has growth changed over time?” A BPP asks, “Does the baby show reassuring signs right now?”


Many pregnancies never need a BPP. Others need one once. Some need repeated testing weekly or twice weekly, depending on the condition being watched.
